We are contracted providers with the following:

• Traditional Indiana Medicaid

• Anthem BCBS (including Anthem Hoosier Healthwise and Hoosier Care Connect)

• MHS Indiana

• MDWise Indiana

• Caresource Medicaid

Insurances are subject to change so please check with us to determine if your insurance is still included.

We also offer competitive private pay rates and accept cash, checks, and all major credit cards, including HSA/FSA cards! Payment for private pay, copays, and co-insurance due at time of service. Charges also apply for cancelations outside of our 24-48 hour window of notification. 100% of session fee with less than 24 hours notice and $50 within 24-48 hours of cancelation. This does not apply to those with Medicaid.

Once we have insurance authorization, we can schedule the initial evaluation and work together with your physician if we feel further services are needed.
